Once you locate an appointment that meets your current needs and you click the corresponding link on the Make an Appointment screen, the Schedule an Appointment screen will appear.
Why are you coming in?: Click this field and enter the reason for your visit here. This is simply a description for why you need the appointment, but is required.
Is there anything else you would like to include?: Enter any other information that you feel is relevant to your visit.
Contact Information: Edit your contact information as necessary.
You may need to click the Refresh button on your browser to show the changes. If the system has been configured to do so, you will also receive an email notification for the scheduled appointment.
